CSV writer example

Example 7.7.2 shows output file open with mode 'wt'. In fact on windows this causes records to be separated with /r/r/d causing confusion for downstream software, like notepad. Per the documentation (sec 13.1) "If csvfile is a file object, it must be opened with the ‘b’ flag on platforms where that makes a difference."
